As a J-1 exchange visitor participating in our program, there is an easy way to report changes to us. Simply fill out the form below within three days of changing your residence or phone number.

If you don’t possess a personal phone number, please use the phone number of the hotel or rental office. For hotel stays, please include your room number.

In addition, please use the form below to report any changes referring to your training program and your host company.
